THINGS TO KNOW

REPORT OF WELL-BEING
• You are always REQUIRED to submit the cover page with your well-being report.
• As a legal guardian, you must be aware of your ward's medical care.
• Make sure to personally answer every question, even if your ward is receiving care in a facility.
• Do not refer us to attachments in your answers.
• The 'Certification by Examining Professional' should be filled out by any doctor who has treated your ward within the last reporting year. No other medical documents are required, and please refrain from attaching IHP reports.
• Remember that you can use a blank Certification by Examining Professional during any appointments over the next year for next year's reporting.

ACCOUNTING
• Statements are not required unless requested.
• Kindly label your accounting with your reporting range dates.
• Ensure that this year's beginning balance corresponds to last year's ending balance.

CO-GUARDIAN REMINDERS
All co-guardians are required to submit a report. Co-guardians have the option to either collectively sign a single report or submit individual reports. In either case, a separate cover page is necessary for each co-guardian. (Co-guardians who reside together may use a single cover page with all names included.)
