The Monmouth County Clerk's Office provides identification cards to U.S. citizens who are residents of Monmouth County.


21 YEARS OF AGE AND OVER

  1. Certified birth certificate with raised seal (this is issued by the town in which you were born, (hospital record is not acceptable);  naturalization certificate; permanent resident card
  2. Proof of Monmouth County residency (valid driver’s license, mail, utility bill)
  3. Proof of social security number (card, W2 tax form, pay stub, bank statement)
  4. Fee is $13.00 – cash, money order, Mastercard, Visa
  5. The hours for an identification card for Monmouth County residents 21 years of age or over are 8:30am – 4:00pm

 

UNDER 21 YEARS OF AGE

  1. Original birth certificate with raised seal (hospital record is not acceptable), or naturalization certificate
  2. Proof of Monmouth County residency (valid driver’s license, mail, utility bill)
  3. Proof of social security number (card, W2 tax form, pay stub, bank statement)
  4. All applicants under 18 years of age must be accompanied by a parent or legal guardian (with guardianship papers).  Guardian and minor must share residence.
  5. Fee is $13.00 – cash, money order, Mastercard, Visa
  6. The hours for an identification card for Monmouth County residents under 21 years of age are 8:30am – 4:30pm
