Permit Application

Thank you for taking the time to apply for a tree permit. This process will save all of us time and money.

In today's world, the cost of doing business is going up. At the Monmouth County Division of Shade Tree, we recognize this and are now offering this "E"(electronic) service to contractors and utility specialists.

Instructions for the Application Procedure

1.) Check print preview settings.

2.) Click on the PDF link to view the form, or simply scroll down on this page.

3.) Print out the form.

4.) Fill in all blanks. Any un-answered questions may delay the application process; for expedient service please complete the entire form.

5.) The application must be signed by an authorized representative of the company doing the actual work.

6.) Fax the signed copy of the application to our office for review.

7.) An approved copy will be faxed back to your office.

8.) A copy of the signed permit by a county agent shall be on site with the contractor for inspection while work is being conducted.
